Who We Are

The Greater Peterborough Health Services Foundation is a non-profit organization dedicated to improving primary health care services in our community. Formerly the St. Joseph's Care Foundation, we work in close collaboration with Primary Health Care Services of Peterborough to support the local Family Health Teams (see diagram below) in the City and County of Peterborough.

What We Do

The Greater Peterborough Health Services Foundation supports the local family health teams by funding equipment, education and research, while fostering preventative action and promoting healthier living for all.

Who We Support

Each Family Health Team is unique. It will vary in size and composition, depending on available resources, the needs of the community, and the number of providers available to participate. See sample Family Health Team model below.

A team approach is useful, especially when patients have complex needs. When a Family Health Team includes a number of experts, it broadens the scope of health care available to support each patient.
